Students name Students grade Guardian name, email, and emergency phone number- for kid students I give permission for all attendees and their art to be photographed by All the Arts for All the Kids Foundation to be used on for marketing and on their social media platforms. If a student's behavior at any time threatens the immediate safety of him/her ,other students, or staff, the parent/ guardian, will be notified and expected to pick up the child immediately. If the the unsafe behavior continues after the parent's involvement, the child will be removed from the class permanently. A prorated class fee will be reimbursed, if applicable. As the guardian, I have read this policy and and agree with these terms. Yes, I understand the behavior guidelines. No, I do not agree with the behavior guidelines. In the event of an accident or injury I assume responsibility, and release All the Arts for All the Kids Foundation, all owners and affiliates of any and all claims of liability.
