Student Artist Name Student Artists Grade Parent guardian name, email, and phone number for emergency If student's behavior at anytime time threatens the immediate safety of him/her ,other students, or staff, the parent/ guardian, will be notified and expected to pick up the child immediately. If the the unsafe behavior continues after the parent involvement, the child will be removed from the class permanently. A prorated class fee will be reimbursed, if applicable. As the guardian, I have read this policy and and agree with these terms. I give permission for my student artists and their art to be photographed by All the Arts for All the Kids Foundation to be used on for marketing and on their social media platforms. Anything we need to know to help us with your Student Artists? allergies? medical conditions? glasses? has a FSD Aide during the school day? Who will be picking up your student artists after class? If you child gets to Encore/LAB please advise that team of the change If there is no school for FSD, there is no AtAFoundation class. OK I understand minor scraps and cuts are part of working with glass, and will not hold All the Arts for All the Kids Foundation liable in minor incidents involving glass during this opportunity.
