Student Artist Name Student artists Grade in the Fall Guardian name, email, and phone number for emergency Anything we need to know to help us with your artists? allergies? mobility? medical conditions? glasses? has an Aide during the school day? ADA requirements? If a student's behavior at any time threatens the immediate safety of him/her ,other students, or staff, the parent/ guardian, will be notified and expected to pick up the child immediately. If the unsafe behavior continues after the parent's involvement, the child will be removed from the class permanently. A prorated class fee will be reimbursed, if applicable. I give permission for all attendees and their art to be photographed by All the Arts for All the Kids Foundation to be used for marketing and on their social media platforms and website. Please give Mr. Hall permission to take the artists out of the studio, on the city block between Wilshire, Pomona, Commonwealth and Harbor. Yes, I give permission for my student artists to walk within the city block to scout out film locations. No, I would rather my student artists stay within the AtAFStudio property lines.
